We’re glad to share that Forum Health is bringing our Brookfield and Mequon clinics together under one roof at our Mequon location. Beginning June 12, 2026 our Brookfield clinic will close and all care will continue in Mequon.
This move lets us combine our two teams, share resources, and offer you a more complete care experience in a single, well-equipped location. The providers you know and trust are coming with us, so the care you count on stays exactly the same just in one place.
What this means for you
