Do Not Sell or Share My Personal Information

At Forum Health, we respect your privacy choices. California law (the California Consumer Privacy Act (CCPA), as amended by the California Privacy Rights Act (CPRA)) gives you the right to opt out of the sale or sharing of your personal information.

 

Although Forum Health does not sell personal information that directly identifies you, we may sell, license, or distribute de-identified or aggregated data for research, analytics, and commercial purposes. Because some states define “sale” and “sharing” broadly, we provide you with the ability to opt out.

Your Right to Opt Out

 

You have the right to direct us not to sell or share your personal information. Once we receive and verify your request, we will honor your preference.

 

What This Means

 

  • Personal Information: We do not sell personal information such as your name, phone number, medical history, or treatment records.
  • Anonymized/De-Identified Data: We may share information that has been de-identified so that it cannot reasonably be linked back to you. You can request to opt out of this sharing where applicable by law.
  • Advertising & Analytics: We work with partners (such as Google, Yelp, and Zocdoc) who may collect data from our site for advertising and analytics. You may also manage your cookie preferences separately through your browser or our cookie settings tool.

How to Submit a Request

 

To opt out of the sale or sharing of your personal information, you may:

 

  • Email us at privacy@forumhealth.com with “Do Not Sell or Share My Personal Information” in the subject line.
  • Call us toll-free at (888) 558-6980.

 

We will verify your request before processing. You may also authorize someone to submit a request on your behalf.

Additional Privacy Rights

 

California residents may also:

 

  • Request access to the categories and specific pieces of personal information we collect about you.
  • Request correction or deletion of your information, subject to legal requirements.
