Carolyn A. Leitzell, MSN, FNP-C, is a board-certified Family Nurse Practitioner with more than a decade of experience in emergency medicine, primary care, and integrative and functional medicine. She brings a deeply personalized, root-cause-focused approach to Forum Health Cary, helping patients achieve optimal wellness through evidence-based treatments and sustainable lifestyle strategies.
Carolyn has completed extensive training through the Institute for Functional Medicine, including APM certifications in cardio-metabolic health, immune health, gastrointestinal health, hormone health, lifestyle medicine, and environmental health. She is dedicated to empowering individuals with the education and tools they need to take an active role in their health.
With a warm, attentive, and collaborative style, Carolyn partners with each patient to create tailored plans that support long-term healing, vitality, and quality of life. Carolyn specializes in root-cause functional medicine with a strong focus on hormone health, gut restoration, metabolic and cardio-metabolic wellness, immune and inflammatory conditions, and environmental health.
