Milani Philip, FNP-BC, started her career in medicine, in 2007, as a CNA in the orthopedic unit before obtaining her BSN and moving into the orthopedic/surgical care center. After obtaining her MSN degree and FNP certification from Olivet Nazarene University, she worked in psychiatry, caring for individuals in underserved communities and long-term care facilities. Milani’s diverse background has granted her experience in treating patients with substance abuse and severe persistent mental health disorders, including depression, anxiety, OCD, schizophrenia, bipolar disorder, trauma and PTSD.
Milani chose to practice functional and integrative medicine because she has always had a passion for finding the root cause of her patient’s health conditions. She believes there are many aspects providers have to take into account when caring for patients. Factors such as; patient’s environment, lifestyle, genetics, exercise, diet, and sleep patterns are vital when creating a treatment plan.
Milani lives by the motto: “treat others how you would like to be treated” especially when it comes to her patients. She enjoys working collaboratively with the patient and others involved in the care their care to provide the best treatment plan. Milani strives for the best health outcome for her patients and is happy to work with Forum Health, where her care standards are shared and celebrated.

Shandalla believes that the healing arts chose her. She was initially an Environmental Science Major until an encounter with an acupuncturist to manage her back pain completely changed her life. What type of insurance does Forum Health Bloomingdale accept?
Forum Health Bloomingdale proudly accepts most PPO plans to provide flexible and accessible care. While we do not accept HMO plans, secondary insurances, or third-party payors such as worker’s compensation, our team is happy to provide a walk-out statement to help you submit claims directly to your insurance provider.
Does Forum Health Bloomingdale accept Medicare and Medicaid?
No, by contracting with us directly, federal law prohibits you from billing Medicare and therefore secondary insurances generally do not apply.
What PPO insurance plans does the clinic accept?
We accept most PPO insurance plans:
- Blue Cross PPO, Blue Advantage, Blue Choice
- Aetna PPO
- Cigna PPO, PPA, & PPA plus
- United HealthCare PPO
We highly recommend you contact your insurance company or plan to confirm in-network.
Do you bill HMO insurance plans?
No, we do NOT bill HMO insurance plans, Medicare, Medicaid, secondary insurances, or third-party payors, including workman’s compensation.
- Payment in full is due at the time of service unless we are contracted with your insurance.
- Copays are due at the time of the visit.
- Patients will receive an explanation of benefits from their insurance company, outlining payments, discounts, and any patient responsibility. The patient portion is due immediately, regardless of receiving a bill from our office.
- At the end of your appointment, you will receive a walk-out statement to bill your insurance (if we are not contracted).
Note: We do NOT accept insurance for our Medical Weight Loss Program.
What does the Physician Assistant do?
Physician Assistants (PAs) are licensed to perform nearly all the same functions as a doctor. At our practice, our PAs fully align with Dr. Epperly’s philosophy on integrative healthcare, hormones, and functional medicine. They have been personally trained by Dr. Epperly to ensure seamless and consistent care for our patients.
Does Forum Health Bloomingdale offer IV therapy?

How do I transfer records to Forum Health Bloomingdale?
Contact your previous physician’s office to learn about their record release policy. Most offices require you to complete a records release form, which they will provide.
How do I get a copy of my records?
You will receive a free copy of your office notes and lab results during your visit. For additional copies, a fee applies, and you must complete a records release request form, available from our office.
Does Dr. Epperly act as the sole physician?
Dr. Epperly serves in a consulting role and is available only during scheduled appointments within office hours. Our team also comprises of certified physician assistants and nurse practitioners. View our medical team here.
We recommend having a primary care provider for urgent care and after-hours needs. Our office does not provide “on-call” services.
How do I become a new patient?
A referral is not required to become a patient. Contact our office at 630-893-9661 to schedule a new patient appointment. We will enter your information into our system, send you new patient forms, and place you in the “pre-scheduling” column. Once your forms are complete, we’ll confirm your appointment with a provider. Please provide your insurance card and credit card information when scheduling.
What happens if I can’t make my appointment?
To reschedule or cancel an appointment, we require at least two (2) business days’ notice. For example, Wednesday appointments must be rescheduled or canceled by the previous Monday to avoid a $75 late cancellation fee. No-show appointments also incur a $75 fee, which must be paid before receiving future services. Cancellation and no-show fees are not covered by insurance. Thank you for your cooperation.
