Forum Health is proud to serve southeast Wisconsin with it’s Mequon location with functional and integrative care providers, health coaches, and a result-driven team.

Dr. Reynolds is dual-board certified in family and integrative medicine. She graduated from the University of Wisconsin-Madison School of Medicine in 1999 and completed her family medicine residency in Milwaukee (Aurora Family Medicine Residency) in 2002. She completed her Integrative Medicine Fellowship through the University of Arizona Andrew Weil Center for Integrative Medicine in 2011 and received her functional medicine certification through the Institute for Functional Medicine (IFM) shortly after that.
