Carolyn A. Leitzell, MSN, FNP-C, is a board-certified Family Nurse Practitioner with more than a decade of experience in emergency medicine, primary care, and integrative and functional medicine. She brings a deeply personalized, root-cause-focused approach to Forum Health Cary, helping patients achieve optimal wellness through evidence-based treatments and sustainable lifestyle strategies.
Carolyn has completed extensive training through the Institute for Functional Medicine, including APM certifications in cardio-metabolic health, immune health, gastrointestinal health, hormone health, lifestyle medicine, and environmental health. She is dedicated to empowering individuals with the education and tools they need to take an active role in their health.
With a warm, attentive, and collaborative style, Carolyn partners with each patient to create tailored plans that support long-term healing, vitality, and quality of life. She earned her Physician Assistant certification from South University, in Savannah, GA following her Bachelor of Science in – Health Education and Promotion from East Carolina University. Shelly consistently strives to stay at the forefront of medical advancements and best practices, continually expanding her skills through ongoing professional development and attending industry conferences and workshops.
Shelly offers a holistic approach to patient care, combining her extensive medical knowledge with a compassionate and empathetic demeanor.
