Jocelyn Segal is a board-certified Family Nurse Practitioner with more than three decades of nursing experience and over ten years as a trusted provider in primary and managed care. She now brings her passion for patient-centered, root-cause medicine to Forum Health Tampa, where she leads the clinic’s Hormone Injection Program for both men and women and offers Affordable Hormone Care to make personalized hormone therapy more accessible.
With a deep commitment to helping patients achieve lasting wellness, Jocelyn integrates her extensive background in traditional healthcare with Forum Health’s personalized, functional approach. She works closely with Dr. Shilpa Saxena, a nationally recognized leader in root-cause and lifestyle-based medicine, regularly discussing complex cases and collaborating on treatment strategies to ensure the highest level of care. Under Dr. Saxena’s mentorship, Jocelyn continues to sharpen her expertise in hormone optimization and functional wellness.

INTERESTS & HOBBIES
Jocelyn’s motto is, “Seek and you shall find.” By being persistent and driven to finding answers to anything in life- including your health- you will find a new dimension of living. On that note, she loves exploring ways to improve one’s health- through nutrition, physical activity, environmental, and spiritual. She enjoys cooking and baking (insists on important occasions only) and tries to change recipes by substituting them with healthier ingredients. She loves playing pickleball, kayaking, walking/jogging, and just being out in nature. She is looking forward to spending more time volunteering with her church, community, and is hoping to do a mission trip someday.
Her hobbies include reading – she is still a member of her best friend’s book club of 22 years – decorating and traveling. Spending time with her family – husband Brad, son Justin, daughter Jessica, future daughter-in-law Daelyn, mother-in-law (who is 98!), and 15-year-old Shichon named Cooper – is what gives her the most joy. She feels blessed working with Hormones by Design-Forum Health and is excited to meet you!

Laura Stolmeier, MA, has 12 years of experience as a medical assistant, mostly in urgent care, and is also a certified x-ray technician. Her dedication to helping others came through her personal experiences of observing family members receive improper medical care and her own fight against thyroid cancer. These situations led her to begin learning root cause medicine. She values bonding with her patients and is passionate about helping each one accomplish their wellness aspirations.
