Lab Decoder reviews existing blood test results only. We accept up to five (5) blood-based pdf lab reports per person. Common lab reports that we accept include, but are not limited to: Quest Diagnostics (including Function Health, Superpower, and select Rupa panels), Labcorp (including Lifeforce), Access Medical Laboratories, Vibrant Wellness (select blood tests), Clinical Pathology Laboratories (CPL), American Esoteric Laboratories, Cleveland HeartLab, Boston Heart Diagnostics, BioReference Laboratories, and Athenahealth.

This program is not appropriate for:

Emergency care or prescription refills
Patients without recent lab work available

No. The Lab Decoder Program is a standalone service and does not require enrollment in ongoing care.

What labs can be reviewed?

To participate, you’ll need lab work from the past six months. If you need labs, you can start with Forum Foundation.
How much does the Lab Decoder Program cost?
The visit is $250, self-pay, and FSA/HSA eligible.
