We’re excited to announce that Forum Health Rochester Hills is moving to a beautiful new upgraded location! While our goal is to open the new space around August 1st, we’ll officially leave our current clinic on June 30th.
While we perfect our new clinic for August, we’re keeping your care seamless:
- Pamela Thomas along with weight loss coaches Darlene McGinnis and Heather Chludzinski, will be seeing patients at our Shelby Township clinic
- Dr. Berke and Joey Fattizzo will see patients at our Clarkston clinic
- IV Therapy and Elise Shuman will be available in Shelby Township
- IV Therapy will also be offered in Clarkston for added convenience
- Holly Alderman will be seeing patients at both locations and will be scheduled accordingly
- Eric Smith will continue to provide lab draws in the Shelby Twp. clinic
Virtual Appointments All providers are available for virtual visits, offering continued convenience and access during the transition.
We’ll keep you updated as we get closer to opening the doors of our brand-new location — and we can’t wait to welcome you in!
